A Clinical Study of Thyroid Nodules in Adult Males

Abstract
Although the incidence of thyroid nodules in males is lower than that in female, but the malignancy rate in males is higher than that of females. So, in the cases of male thyroid nodule, there should be careful consideration of malignancy. We
clinically
analysed 107 cases of adult male thyroid nodule diagnosed and operated on at the Department of Surgery, Capital Armed Forces General Hospital, over a period of 11 years from January 1984 to December 1994.
@ES The results obtained were as follows:
@EN 1) The age distribution was 76 cases in the 3rd decade, 20 in the 4th decade and 11 in the 5th decade. The overall incidence of malignancy was 30.8% and the peak incidence of malignancy was noted in the 5th decade at about 54.5%.
2) The duration of illness prior to diagnosis was less than 1 year in 75 cases(70.1%).
3) The most common symptom was the presence of palpable mass in all cases. Others were palpitation, weight loss, headache, and fatigue in order of decreasing frequency.
4) The most common finding on thyroid scan was a cold nodule in 89 cases(87.3%), however thyroid function test showed euthyroid in 90(84.1%) out of 107 cases. These findings show no significant value in differentiating benign and malignancy
nodule.
5) The location of the thyroid nodule was in the right lobe in 61 cases, left lobe in 38 cases and bilateral in 8 cases. The right lobe was more frequently involved.
6) Unilateral lobectomy was performed in 66 cases(89.2%) of benign nodule, and subtotal thyroidectomy or lobectomy in 29 cases(87.9^%) of malignancy one, and modified radical neck dissection in 3 cases of positive cervical lymph node.
7) The most common histopathologic finding was follicular adenoma in 47 cases(63.5%) out of total benign cases and papillary carcinoma in 26 cases(78.8%) out of total malignancy cases.
8) Postoperative complications developed in 15(14.0%) out of total 107 cases; including 7 cases of transient hoarseness, 5 cases of hematoma and seroma, 3 cases of recurrent laryngeal nerve palsy.
